體內(nèi)過程口服易吸收,0.5 1h達(dá)到最大血藥濃度.在常用臨床劑量下,絕大部分藥物在肝臟與葡萄糖醛酸和硫酸結(jié)合為無活性代謝物,t1/2為2 4h.較高劑量時,上述催化結(jié)合反應(yīng)的代謝酶飽和后,藥物經(jīng)肝微粒體混合功能氧化酶代謝為對**苯醌亞胺.**苯醌亞胺是一個有毒的代謝中間體,可與谷胱甘肽(glutathione)結(jié)合而解毒.長期用藥或過量中毒,體內(nèi)谷胱甘肽被耗竭時,此毒性中間體以共價鍵形式與肝,腎中重要的酶和蛋白分子不可逆結(jié)合,引起肝細(xì)胞,腎小管細(xì)胞壞死.

不良反應(yīng)與注意事項Acetaminophen為非處方藥.偶見皮膚黏膜過敏反應(yīng).長期使用極少數(shù)患者可致腎毒性,如腎乳頭壞死和慢性間質(zhì)性腎炎等.過量誤服(10 5g以上),可致急性中毒性肝壞死.

藥物相互作用 ① Aniline與肝藥酶誘導(dǎo)藥如巴比妥類合用時,易發(fā)生肝臟毒性反應(yīng);② Aniline可延長氯霉素(chloramphenicol)的半衰期,并增加其毒性.
