盡管經(jīng)過多年的改良與發(fā)展,如今的電療的安全性、有效性上都已經(jīng)得到了很大提升,成為了重度抑郁癥、精神分裂癥等精神疾病的最后一道防線,然而只需要幾位醫(yī)學(xué)狂人加上幾句親身經(jīng)歷者的證詞,之前一切正面的公眾形象都將不復(fù)存在。相反,關(guān)于電療的爭議或許則將長期存在下去。

1 月 10 日,據(jù)美媒 NBC 的報(bào)道,斯坦福大學(xué)醫(yī)學(xué)中心的神經(jīng)外科助理教授 Casey Halpern 宣布他和他的研究團(tuán)隊(duì)通過研究老鼠和人體大腦中的電信號,發(fā)現(xiàn)使用電脈沖刺激大腦特定部分時(shí)能夠抑制刺激成癮的沖動行為,比如暴食癥、賭癮等。

電刺激抑制成癮?加上之前世界衛(wèi)生組織將游戲成癮列入精神與行為障礙,這難道意味著「雷電法王」楊永信要被成功洗白,名正言順地進(jìn)行他的「電療」?

但是先等等,這些年被楊永信等人「普及」并且妖魔化的電休克治療(Electroconvulsive Therapy,ECT)真的就是一個(gè)邪惡的技術(shù)么?「楊教授們」又是怎樣「掌控雷電」的?

電療簡史

(前方高能,下文中圖片可能會引起不適。)

早在 16 世紀(jì),瑞士內(nèi)科醫(yī)師 Paracelsus 就發(fā)現(xiàn)口服樟腦引發(fā)的癲癇/抽搐可以用來治療精神疾病,但是這背后的原理到底是怎樣基本還處于瞎猜的狀態(tài)。

期間治療精神病的手段也是千奇百怪,比如用鴉片、放血,甚至是把人大腦中主管情緒的前額葉攪碎的「冰錐療法」(Ice Pick Lobotomy)。

直到上世紀(jì)初,人們才發(fā)現(xiàn)癲癇不會和許多精神疾病同時(shí)發(fā)作(這也是電擊引發(fā)抽搐來治療精神疾病的理論來源)。

于是在 1934 年,科學(xué)家再次將目光投回癲癇減輕精神病癥狀上。匈牙利神經(jīng)精神病學(xué)家 Ladislas J. Meduna 用樟腦油引發(fā)的癲癇成功治好了一例 4 年的緊張性木僵(Catatonic Stupor)。之后他使用一種在誘發(fā)癲癇上更可靠的藥劑卡地阿唑(Metrazol)治療了 100 多名精神分裂患者。

然而這種更可靠的藥劑在誘發(fā)癲癇的過程中會讓患者感受到恐懼,甚至醫(yī)生和護(hù)士需要追著患者喂藥。

當(dāng)然看到藥物治療有這種缺陷,人們便著手尋找更「人道」的治療方法——電擊:用電流刺激腦部以誘發(fā)癲癇,這樣比藥物直接更有效。

于是在動物身上做過實(shí)驗(yàn)之后,1938 年意大利精神病醫(yī)師 Ugo Cerletti 和 Lucio Bini 便將這項(xiàng)技術(shù)應(yīng)用到了人身上。

第一名男患者是一個(gè)在羅馬街頭找到的時(shí)常妄想且胡言亂語的人。在第一次電療的過程中,癲癇被順利誘發(fā)了,而在患者清醒之后,甚至邏輯清晰地罵起了人。。。

此外,電擊還有一個(gè)積極的副作用:逆行性遺忘(Retrograde Amnesia),也就是因?yàn)檫@個(gè)副作用,患者不會記起治療的過程以及難受的感覺。于是,電療迅速取代了藥物并開始普及,而 Ugo Cerletti 和 Lucio Bini 也因此獲得了諾貝獎(jiǎng)提名。

之后,電療經(jīng)過了一系列的改良,比如 20 世紀(jì) 40 年代至 50 年代初無抽搐式電休克治療(Modified Electroconvulsive Therapy,MECT)的出現(xiàn),將肌肉松弛劑(Muscle Relaxant)和短效**劑(Short-acting Anesthetic)引入了電療,以防止患者骨折和肌肉松弛劑可能引起的窒息感,「用戶體驗(yàn)」得到提升。

自 2016 年 2 月起,美國的食藥監(jiān)局 FDA 就已經(jīng)考慮將電休克治療的安全性從 Class III(high risk)改成 Class II(low risk) 了;在我國,電休克治療也是治療精神疾病的常用手段。所以說電療的有效性和安全性其實(shí)是有保障的。

然而,為什么電療在公眾心中的印象普遍還是負(fù)面的?

一個(gè)被妖魔化的技術(shù)

在最新的《黑鏡》第四季第六集中,一名被判死刑的黑人囚犯的意識被制作成了一個(gè)數(shù)字影像存放在了博物館里,而這個(gè)數(shù)字影像的「工作」便是供游客拉下電閘并受到無休止電刑的折磨。

事實(shí)上,在很多人的印象里,電療的過程都是上面這樣一種折磨人的存在。而這種印象其實(shí)一部分便是來源于「楊永信們」對于電療的亂用以及濫用:

正是因?yàn)槭艿诫姄魰r(shí)病人會產(chǎn)生抽搐甚至是骨折,所以這種「療法」就被許多精神病醫(yī)院用來懲罰那些不聽話的病人。

而自上世紀(jì) 50 年代起國外媒體開始聚焦這些負(fù)面消息時(shí),公眾對于電療的印象就開始急轉(zhuǎn)直下。在《飛躍瘋?cè)嗽骸罚∣ne Flew Over the Cuckoo's Nest)里,電療便成了一個(gè)邪惡護(hù)士長手中的恐怖工具。

此后,電療更是被用在了同性戀的「掰直治療」上?!短禊Z絨金礦》(Velvet Goldmine)里滿臉笑容的護(hù)士或許是真的以為電療能「幫到」同性戀者吧。

時(shí)至今日,試圖用電擊來「矯正壞習(xí)慣」的錯(cuò)誤做法仍在世界范圍內(nèi)發(fā)生。

去年 4 月,據(jù) BBC 報(bào)道,印尼一所小學(xué)的**就要求 4 名學(xué)生輪流站在一塊通電的板子上,并在學(xué)生的額頭上放置感應(yīng)片,讓電流通過學(xué)生的身體。而**事后更是辯稱電療有助學(xué)生左右腦平衡,這樣他們就不會搗蛋了。

說到底,以上這些瘋狂的行為并不是真的為了用電擊引發(fā)癲癇來治療精神疾病,而是純粹為了帶來肉體上的痛苦讓「患者」在精神上產(chǎn)生恐懼并最終屈服。

「楊永信們」的做法不僅造就了一段黑暗的歷史,更妖魔化了一個(gè)本用來治病救人的技術(shù)。
